Your first visit to our office is the most important one. It gives both of us an opportunity to get to know each other and establish a strong relationship that is important to us in achieving our common goals. This time together will allow us both to determine if our approach to dental health is appropriate and comfortable for you.

In some instances, your first visit to our office may be is for an emergency or a specific concern. In this case, we will address the emergency situation or concern first and make you comfortable.

Once you confirm an appointment with our office, we will send you a patient portal link to fill out your paperwork online. If you’d like to print and fill out the form at your own time, click here.

Every 6 months, we need to update your general patient information and your medical history. You can simply click on the link below to download the form, print it out, and bring it completed to your next appointment.

Tips for making your first visit smooth and successful:

Schedule your appointment with us by requesting it online or giving us a call.
Bring your insurance card, if you have dental insurance.
Once we confirm your visit we will send you a link to fill out our new patient forms online prior to your appointment. You can also download and fill it out from here. This will save you some time during your appointment.
Once you arrive, please check-in at the front desk and provide your information as needed.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

Insurance & Payment

Having a Healthy Smile, Now is Possible!

You shouldn’t have to weigh insurance coverage or climb financial obstacles in order to obtain premium dental health care. It is to your advantage that you are able to reserve an appointment for service right away.

We accept all major credit cards, ATM cards, cash, and personal checks. For our patients’ convenience, we have arranged a payment plan through a third party, CareCredit®, and in-office financing if approved, which helps our patients receive their treatment in a timely manner.
